I understand I am responsible for myself / my child throughout all classes and I / my child are not in the care of ‘Music Lessons With Rose Pty Ltd’ at any point in time.

I have read, understood, and agree to the fees, class types, and membership tier I am enrolling in.

I understand that lessons are paid in block payments for memberships of a year, semester, term, or casual and payment is required upon receipt of the invoice, prior to class commencement.

I take full responsibility for all class payments, including casual class attendance, which must be paid upon booking or receipt of the invoice.

If I am enrolling on behalf of a child who is not in my guardianship, I understand that by completing this form and listing myself as the primary contact, I am accepting responsibility for any associated accounts.

I understand that I may be refused entry to class if the invoice is not paid in full or if no payment plan has been arranged.

I understand that my booking is not secured without payment.

I understand that payment plans are only available for year memberships, incur a $30 administration fee, and require half the fee to be paid immediately to secure the booking/membership, with the remaining half due within one month.

I understand that all learning resources are separate payments from class bookings, and payment for these is due upon receipt of the invoice.

I accept responsibility for payment of any books and resources purchased for myself/my child.

I understand that a term booking reserves a time slot in a class for myself/my child for weekly attendance, excluding any other student bookings.

I understand that 24-hour notice is required for any classes I/my child are unable to attend.

I understand that classes are limited to the times offered, and no alternatives are guaranteed for missed classes as a rule.

I understand that if I/my child is unwell, with presenting symptoms or not, I will cancel the class or arrange to have an online class.

I understand that a class may be cancelled or entry may be refused due to illness, at the discretion of the staff member, to protect the health and safety of the staff and student community.

I am aware that online classes are available for convenience and flexibility, and I take responsibility for giving appropriate notice to arrange this and organising the home set-up for this.

Studio memberships are binding, non-refundable, and remain active until their expiry date.

I agree for ‘Music Lessons With Rose Pty Ltd’ to use my details for class bookings and subsequent subscription updates, which I can cancel at any time.
